Sorry, I miss a problem:
4- Left and right input signals are "mixed". If I insert only right rca there's sound also in left channel and vice versa.

With the gain pot in it's center position, measure the resistance from the two pins on the pot nearest IC1 to the RCA shields (no RCAs plugged into the jacks).
Now gain work, I don't know why last time it didn't work.
Resistance are: 17k and 5.7k. The first one change when I turn pot.

Does the gain for the two channels match if you drive a sine wave into the amp and measure the AC voltage across each channel?
