Yes i use 10uf x7r and i use screen shielded cable so close to load. I dont use big output cap on my project.
You have said “i need to more then 5a” 4 quad chip 2 amper. You need to use 10 chip for 5A and it could be very expensive…. If you dont desing so specific devices that s so sensitive to noise.
I use my 78xx and 79xx pin out v- and v+ regulator if i need to more low noise

You can build lt3042 npn high current version with lt3045 and D44H11 but carefull about lt3045 pins. If you need to Continuous more than 5A always. you can try BUV48A but I not sure I havent tested it. input voltage must be 16.5-18v and you need to really big heatsink. Please create good ground plane for lt3045...
you are right buv48a not good about hfe... d44h11 has 50-60 hfe. it is good choice. for lt3045. 2sd1047 is the same but end of life.

it is depend on where you use it. I use it on my es9028prodac. DVCC paralel 1A , avccl and avccR 500ma and crystek 100mhz 500mA. I cant say about sound but Fast Start-Up Capability Load Transient Response are very good. I dont thing use it on my pcm56 dac. definitely I will use LT3097 on my discrete IV stage...
If you use a super regulator you dont need to RL filter. My experience is R Or L filter better for smps and class a source…

Yes I have seen a lot of brand make it. Especially marantz. But you dont make discrete power supply. you want to make LT3045. High psrr ultra low noise etc.. please read LT3045 datasheet. you dont need to additonal filter etc.. Unnecessary filter sometimes can effect high freq. bad on audio devices
