@PierreQuiRoule I got your feedback and increased the spacing. I will make a quick order with Osh-park and see that they work as expected, if so I'll make a larger run and sell that at (pretty much) cost
Ground planes on both sides, connected at the various GND pads.
The input/output and 2-ch GND's are all connected. Any feedback welcome if there are more optimal approaches.
I would recommend that the ground planes NOT be electrically connected to the mounting holes.
Allow plenty of clearance space for the nut and the metal surface on the other side of the board,
so there is no chance of contact between them and the ground planes on both sides.
Otherwise, ground loops can be formed.
@rayma The mounting holes are not electrically connected (there is no pad there, just a hole in the PCB).
I increased the the clearance sufficiently and I think it should be good now.
I did one additional change though and that is to not connect the GND of both channels, in case the design requires that they be combined elsewhere. (IN/OUT is still connected though)

Got it. Those were additional GND connectors, with the intention to be used if you wanted to connect both channel GND to a single point, but I removed them since I separated the GNDs of each channel
No problem folks. I ordered a small quantity from OshPark to ensure the physical dimensions (I created the footprint using a caliper so could be off potentially).

OshPark order took a while. I got them yesterday and fit was 95% but not perfect. I made adjustments and ordered a batch from PCBWay (Black PCB with HASL) which I expect to receive in the next week or so. Please PM me directly if you're interested.

They will go in a bubble mailer so I estimate around $3-$4 for USPS First Class.
