SMH series
Digi-Key
has up to 100V
Digikey and Mouser (and others) list 160V but don't have any.
Search in www.findchips.com for SMH161.
Try contacting Digikey and see if they can order them?

On capacitors that big, many times the extra pins are to hold it down to the board and keep it from breaking off. check to see which pins are connected to each other and to case . I don't think you have a 5 section capacitor like in the old tube days.
If you can't buy caps with the extra pins (I've never seen them in stock) then you make up for it by gluing the cap to the board with wallboard adhesive or silicon seal or something.
One source of extra pin caps is Peavey, for the ones in their actual amps they built with them.
