Extremely noisy amplifier.
Hi!

In order to make measurements in class-D amps, you need a low pass filter so as to just let audio frequencies pass.
I suggest a 20kHz 2nd or 3rd order low pass.
In addition, if your amp works in bridge mode, you need a differential pre-amp or a differential probe since none of the output leads are common ground.

Also check the power supply at the input of the amp if it is not oscillating. If yes, just decouple with capacitors.

The circuit below is what I used once I needed to take some measurements on bridged class-D.
You can improve the filter to higher orders so as to remove as much as possible the high frequencies.

It decouples DC, reduces the voltage by 10 times, implements a differential input and a 2nd order low pass filter.
