I'm looking for help identifying a 5-pin SMD component. U10: A04
The driver board is from a 4-channel Nemesis.
The problem is that the IRS20124 was replaced and a shorted output transistor.
When powered on, the audio output FET quickly shorts out.

I don't know exactly what you should have except for the output of the op-amp being the same as the input on pin 3.
With no outputs, the servo loop is open and the op-amp swings to either power supply rail. Closing the loop locally (the 1-2 bridge), allows the op-amp to see what it's looking for.
Do you have a limiter you could insert in series with the high-side drain to help protect the output FETs? A 120v incandescent lamp would be one of the better limiters.

Replacing the NE5532 solved the problem.

I only have a current limiter on the 12V input with a set of resistors with a 2-ohm impedance.
