Amp powers up and produces rail and regulated voltages .
Amp has no output .
I do not get any rail to rail oscillation on the outputs .

If I adjust the gain on the amp the square wave cleans up but still a square wave on the input pin

Repost the list of voltages that you did previously but use two references. For all voltages ±20v, use the primary ground. For all others, use the negative rail.
Put this in two different lists.
Leave the ground on the negative speaker terminal.

So use ground terminal for voltages above 20 volts and use negative rail for voltages 20 volts and below ?
There will be voltages that are near ground (within ±20v). Those are referenced to the secondary ground (which is currently tied to the primary ground).
The voltages that are near the negative rail are referenced to the negative rail so you have to use the negative rail as the reference.
