Look at Osh Park for smaller boards, and Advanced Circuits $33 special for larger boards.
This. Best bang for buck.
I am on my 350th order with JLCPCB.
Only ever had one problem.
I was advised one pcb got damaged during manufacture.
I didnt think for one second they would send it to me and it got sent out to a customer !
It had a random 6mm hole through the middle of it.
JLCPCB are cheapest.
I tend to go for cheap postage too, takes a good few days but worth waiting for due to excellent quality.

I've been going osh park. US, ENIG finish standard, a little more expensive, but for small boards not that much more. If I had a big board, say more than 10sq in, I'd look into other options, but the 6 or so designs I've done so far have all been less than 10sq in. Osh's web interface is super easy to use as well.
Not sure, seems like you're missing a layer somewhere. Did you use the export template JLC provides for eagle? https://support.jlcpcb.com/article/137-how-to-generate-gerber-and-drill-files-in-autodesk-eagle
