I'm building a 1/4" TRS headphone extension cable, and have a nagging question. Does anyone ever leave the shield floating at the headphone end, the way it's sometimes done in power cables? I assume not, but I just got to wondering.

The shield should be connected at both ends.
Headphones don't need a shield. (Signal level is high enough to overwhelm most external interference.)
Anyway your head is not a good ground like another amplifier box, no ground loop.
The Sleeve is the common return. Without it the sound is a mono mix.

It's better to utilise the ground wire/connection of headphone cables/connectors to avoid any issues which could potentially damage the sensitive/delicate headphone drivers.
I have lost count of how many headphone cables I have repaired where the ground terminal of one or both channels has failed, usually due to cables being stressed/yanked.
