TPA3116 Sudden Distortions

Hello,
I’ve been running two PS95 on a tpa3116 for like a month but all of a sudden i heard some popping and cracks, and now it always sound distorted. I previously did the 20 dB gain mod but thats everything I’ve done to it.

any idea what could the problem be?

I have also tried a different power supply, resoldering the gain resistors. same problem.
