How about both simultaneously.
A simple example is a word which is common in several languages, but with a different meaning altogether.
(an infant has thoughts, but no language)

An infant already speaks a language, the most basic one.
A general adult speaks 2 : the basic one and the mother tongue.
A bottom telepath will do basic tricks : read your feelings, not your language.
A stage two telepath will read your feelings, and your mother tongue thoughts.
Logic would suggest that the most spooky telepath will be able to feel every feeling, and think in every language imaginable. Simultaneously.
But it's all theory anyway, isn't it.

The aliens aren't Santa, they have limited ability in how many people they can do genetic engineering on, they only choose a few select people to perform their experiments.

Since our brains are accompanied by varied electromagnetic fields (notice I leave out causality here) if we are able to sense them somehow, we can communicated telepathically. Communicating over long distance is somewhat of a problem without a known medium of transmission but perhaps long wave EM waves may be an answer. Also, the heart is also accompanied by large EM fields, according to the latest research.
So telepathic communication is technically possible in that the EM fields accompanying the brain projected outside of the body to within the 'hearing' or 'sensing' range of another person. Now if it were only possible to train a person to recongnize these variations, we are a long way towards getting it done.
