now the board uses DN2540 in TO92 package ... it takes nothing to modify it or maybe add this FET too in the GB

Bas I was referring to the complete pre but not a problem to crop it to a small PCB. I'd prefer to maintain the heatsink on the PCB because FETs need to be isolated and this way thing are much easier and safer. What about the B+ regulator? Included on PCB or not? You need a super clean B+ when using the CCS as mu-follower.

if you look at my schem at post #18 you will see the regulator on the left: it is a CCS shunting a big resistor and feeding the gate of a high voltage MOSFET: the CCS runs at 5mA and sets the voltage at the gate of the MOSFET, the regulated voltage at the source of the MOSFET is 4/5V below the gate voltage. Nothing fancy but it helps in knocking out +50dB of noise. On the right the CCS and tube (that we can eliminate from the PCB). When you use the mu-follower output form the CCS the power supply noise rejection is poor and you want it to be as clean as possible in preamps. So I thought that retaining the regulator on the PCB might be useful. In case you need the CCS for a long-tailed pair or cathode followers (ie on cathodes) the regulator should be bypassed.
