Header bypass Cap?

I built the Aikido Phono Preamp and it work out well for awhile, but then I cleaned up the board, changed tubes and some resistors. Now my heater bypass cap keeps exploding, can anybody think of a reason? Is it a bad tube or is there something shorting? See the user guide below, it's on page 5.

C16 capacitor voltage rating too low? Installed backwards or filament wiring polarity swapped?

I installed a 35V cap and my heater is running at 12.6V and I checked the polarity as well. It was working fine until I changed the cathod resistors and the tubes.
