Anyone remember this board that was part of a group buy years ago? I dug out four of them but couldn't find the info. Does anyone have the info on these?

I took a board and put the component numbers on a tube cad octal schematic just in case there was someone like myself that had some boards laying around that wanted some info. The board that I scanned is for 6Sn7/6SN7 or same pin out.
