Take a look at Arjen Helder HiFi. He has various boards that should meet you specifications.
There are other vendors on eBay that may meet your requirements as well. Do a search for "T-Amp".

Does it have to be class D? Easiest method to get a 50W amplifier might be a LM3875 or LM3886 based, class-AB chip-amp.
Chip-amps are also far more tolerant of layout problems than class D, which means they're much easier to breadboard/prototype. You don't want to build a class D amplifier on a breadboard. Parts acquisition is also a lot easier, as far as finding things like appropriate inductors/capacitors for the output filter.

Search the International Rectifier site. Lots of schematics/App. notes of Class D amps. Some simple...some not so much.

Best chip for CLASS D in very less time is LM3886. U can wire it on breadboard and it works. Very cheap PCB are available in market . Output is great compared to market.
LM3886 is not a class D device at all. It is a quasi class AB power amp on a chip. Also it runs hot, especially if you push it hard.
