I've searched the forum and net but couldn't see a definitive answer, so please bear with me for this naive question, can someone show me a clear instruction how to wire the CMLI-15/15B for unbalanced input, balanced output? I will use it in front of AMS1000-2600 class d amp.

Did you check the datasheet?
Transformers do not care if the source or load is single-ended or balanced.
Here's a quick sketch. Just mind the phasing (brown is in phase with orange).
The CMLI-15/15B should be terminated on the secondary with 10K for ideal frequency response. Isolate the RCA from chassis.
The folks over at Cinemag will be happy to help you out with specific applications (e.g. if you need something that has very high impedance).
