Welborne Labs Ultrapath BP preamp. A nice little preamp, included 2 sets of tubes, the Amprex are unused NOS.

The batteries are long since dead, but are commonly available sealed lead acid. Replace the batteries, and should work fine.

Manual (assembly/owners) included.
