If you really need the DIP-14 footprint it's probably best to make up an adapter for two dual DIP-8 op amps
The TL084 is about the nearest to what you are looking for.
LM837 is closer (but still not very close) - at least its bipolar.
Isn't there a 4-channel LME49740? The LME49720 is basiccally a rebranded LM4562 so this is probably exactly what you need..

There was. I think the best (in terms of specs) Quad Bipolar Input audio OpAmp available today is the OPA1604.
Edit: There is also a quad version of OPA1642, called OPA1644. Depending on the application I would use one of those two.
And both of them are SMD only. So you would need an adapter either way..

Thanks, Jan---that was it, the LME49740. Unfortunately no longer available. I'll just take DontHertzMe's suggestion and use 2 x 4562s.
