I bought the PCB and all the parts on Mouser and the batteries on eBay a few months ago and I just never got around to putting it all together.

Kit is $69 from JDS Labs and doesn't include batteries, wall wart, volume knob or shipping.

The parts I have includes everything in the JDS kit minus the 2 headphone jacks, I was going to hardwire mine to some good quality inputs and outputs for a permenant setup. Also includes batteries, wall wart, and volume knob.

$59 shipped to the US!
