If you have, or can borrow a copy of Morgan Jones' Valve Amplifiers, there is an explanation of the m-follower/m-stage circuit from pages 115 to 120 (in the third edition).
I found it somewhat easier to comprehend than the Kimmel article (which is mainly concerned with the use of a pentode or MOSFET as the follower), but of course the Jones article isn't as easy to get your hands on.
