ICEpower 1000asp DIED....

Last week I discovered that my ICEpower 1000asp amplifiers (two monoblocks, Model 501's, by Jeff Rowland) were DEAD. UGH. Before I put two new modules back into service, I would like to ask what forum members might think caused it, and how to stop if from happening again.
FIRST - I will fully admit I 'drank the kool-aid' and though these were robust enough to plug directly in the wall and leave on 24/7 (as indicated in JRDG the literature). So - I am employing a whole-house + local outlet surge supression strategy (with voltage monitoring).
SECOND - in the "DEAD" modules, (while on the phone with Jeff Rowland - he was helping me troubleshoot) I could apply power and see the status LED flash green, on the ICEpower module. It did not flash in a rythmic fashion, more 'a-rythmic'. It was flashing green. He had never seen that before.
Last bit if interesting info: the rest of my rack was not connected to the power during the 4 days that this could have happened. BOTH monoblocks exhibit the same behavior, so I can rule out a 'naturally' malfunctioning board.
FINALLY - Is this harmonics on my power line? Was it a surge? (no other items in my house fried) Are there known issues that cause that LED to flash at random intervals?
 
I don't think it was the soft clip LED, it was the 'on board' green/orange 'status/power' LED. It should be on 'solid' green when everything is OK, or Orange is there is an error (according to Jeff). I wish there was a tech support line at ICEpower! I sent an email, but no answer.
