Hi elnec,
I'm not quite sure what you are after. But if it's a free-standing HD I would go for USB. Search the TI website, they have some nice USB-interface chips. If the TI chips don't support I2S you will need to go via SPDIF.

This would involve designing an embedded computer system and writing software to control the flow of data from the hard drive to the output.
Why not using the existing solutions, numerous hard drive/network players, mostly based on linux? I kind of do not understand the question. If chips on the sound card (PCI, USB, PCI-e, FW) expose I2S lines, it is just a question of tapping them and transfering somewhere else via some proper channel (e.g. LVDS).
