Is it possible to switch between rail voltages using a switch that jumps a diode on the transformer primary? I have a transformer that yields +/-38v rails and most of the time I use the amp at reasonable volumes (within 19v rails) and I'm looking for a way to switch to the lower voltage. This should reduce heat dissipation at low levels.

You can't place diode in transformer primary. This will introduce huge amount of harmonics and DC offset. Better solution is switching primary 115-230 (if your transformer has dual primary and voltage in your country is 115 V). Also, amp must be designed to work correct with both PS. Another (simple) solution is switching secondary voltage (only for current amp part) if you have taps on secondary.
Some complex amps use PWM PS with variable output voltage(Sunfire....)
