I repaired a Behringer Eurolive B212D active speaker with two blown TVS diodes about 6 months ago.The only issue I saw was D4 and D8 in the voltage doubler(?) being obliterated, everything else looked good other than some baked capacitors. I assumed this was probably due to a power surge on the mains, so I replaced all the small caps and the damaged diodes and all seemed good. It's back wth the same two diodes blown up again. Does anyone have any insight into what could be causing this?
Most of my previous experience with switch mode supplies has been just throwing them in the garbage and replacing with new, but this is integrated into a 2 channel amplifier board so it's not really possible to do. I've attached a schematic for a B215D which looks pretty much identical.

I didn't sign any NDA's to download the schematic, it's all over the internet but it looks like it's not meant to be.
