If a transformer has to be designed for two different primary voltages then it is best if they are in a ratio of 2:1. Then two primaries (i.e. 4 wires) can be used in parallel or series. This gets the best ratio of copper to power handling.
A 3 wire primary means that either the transformer must be derated on the lower voltage or contains more copper than needed for the higher voltage.

not as efficient as DF96 says, but it can be done.....
for small power applications, like 5 to 50 volt amperes, this is not usually a problem...
for higher powers, not so good...
A dual primary transformer is a good way to make a "universal" transformer for international use.
Two primary windings rated for 110/120Vac each allows one to use the transformer on a variety of mains voltages from 110Vac to 120Vac, or from 220Vac to 240Vac.
If the core is rated for 50Hz rather than 60Hz then it becomes a truly universal mains transformer where it will work anywhere except the 100Vac in Japan.
A 3wire "tapped primary" is not as universal.
