ICEPower 300AS1

Hello there,
I've built a guitar amp using an ICEPower 300AS1 module for the power amp. The pre-amp is a simple op-amp circuit, which is powered from the ICE module's 12V rails. The amp worked great until today. Today, the two status LED's from the ICE module are flashing quickly and there's no output from the ICEPower module. Checking voltages indicate that the 12V rails are at 1V. The 54.5V rail is good. Any thoughts on what could have happened to the 12V rail and why the LED's are flashing? Flashing is not discussed in the product datasheet. Thanks for the help. Cheers.
 
I was thinking that a fuse had blown, but couldn't verify. I've seen other posts about a fuse labeled F300 on other ICEPower modules, but the 300AS1 doesn't seem to have a component with that designator. I could be overlooking it however.

When disconnecting either the + or - leg of the 12V rails from the pre-amp, the status LED's go back to a normal state. So I'll tear down the pre-amp and see if something is goofy there. That would be strange since it's just a simple TL074 circuit that's been working fine for weeks. Thanks for all the help. I'll let you know the results of the pre-amp teardown.
