Mouser has a suitable Form A relay, but the amplifier uses both Form A and Form B relays. The Form B is non-stocked with a ridiculous minimum order quantity, so I need to find an alternate supplier. Tried Newark and Digi-Key, but neither had them or their search system was so bad I couldn't find what I needed.

This JWD-171-17 form B relay has the same 500Ω coil resistance as the DIP05-1A72-11D, but has a lower contact rating.

Holy smokes, those are expensive! The Jameco one looks like the best option, cost-wise. The description says "1 Form B contact" but the data sheet for that -17 part shows "1 NCO contact". Not sure what to make of that.
JWD-171-17 seems a good choice, on Mouser too. Has diode, same pin out, 500 ohm, 500mA, only 10W max power though.
It might be the case that a SPDT relay could also serve (depending on precise pinout used). (With reed relays single NC contacts are unusual as a magnet has to be built-in to mechanically bias the contact closed - the winding cancels the magnetic field). SPDT relays use one magnetic and one non-magnetic contact.
