Secondary wiring w/ 2 different windings

Like this. There is only one 0 volt point designated and that is common to all windings. Also make sure you get the phase of the 12 and 18 volt windings correct. If you measure between the 12 and 18 points you see around 30vac. If you see much lower than that then reverse the connections to either one of the windings, the 12 or 18, it doesn't matter.

Yes, you're right. But since the secondaries aren't symmetric, it seems the 0 in 12-0-18 won't actually be at 0V... Am I wrong on this?

You are wrong. 0V is just a stake in the ground, what other voltages are referenced to. So you have one rail 18V higher and one 12V lower. 0V is NOT the mathematical middle of the voltages.

Also, so you're saying just take the actual signal lead to the op amp, don't even worry about bring the input jacks sleeve/ground?

You absolutely need the ground/screen/sleeve to be connected as that is how the two circuits are tied together.
