Not true, there used to be N channel mosfets with a secondary sensor device on the die common source and gate with separate drain. Obviously the package costs a little more but the die will be almost the same as a standard multicell device

Are you referring to SenseFETs? That was not a diode but a tap on the die providing a fraction of the drain current, so you could measure the current without a sense R in the main D-S circuit.

That's right, I was making the point that adding additional devices on a die is not hard for the chip maker so long as it does not require additional mask and diffusion stages. The only extra cost is bonding and test.
