I just had the same problem.
I was getting massive oscillation on the output then the output went to about -30 volts stayed there.
I think the oscillation fried the 7294 so I just threw the pcb out and am buying a new 7294 to try in a new pcb.
These chip amps are good when working ok but are a pig to desolder.
My pcb was only a couple of pounds so not worth resurrecting.
Just out of interest I removed the 7294 and found output shorted to B-.
So 7294 blown.
