I did not say that the circuit is wrong, I only posted the differences between the ciruit in the datasheets and the one you posted, thats all.
Any way, it seems that you got some thing wrong in the input circuitry here if possible, try to use a function generator, and an oscilloscope, and test all the stages, sure you will find some thing wrong here and there.
