If this amp was on my bench I would change the MIC4420 and the HPCL2211.
99% of the time when a BD1000 or BD1500 has distorted output this is the problem. Not always but most of the time.
I dont want you to keep changing un-needed parts. You might want to wait for Perry's input.
Usually if you have a bad opto-coupler or driver you can tell by metering the outputs from gate to source.
Do all 12 outputs read the same from gate to source with your meter?
If some read different which ones?

From my experience the only pins on U5 that should have voltage are pins 4&7.
U15 should have DC on pins 5&8 and a little on pin 7.
I dont beleive pin 5 on U6 should have any voltage either.
The voltages dont look right on those 3 parts.
Have you compared them to U8, U9 and U10 from the other group to see if your readings are very different?
This would be much easier if you had a scope...LOL!!
