Op amp fault?

G'day all, yesterday evening one of my beloved ESP P06 DIY phono stages developed a strange fault (although it may have been there longer, but I never noticed it!

A loud 'whoosing' noise only on the right channel. I immediately suspected a faulty op amp and yes it proved to be that. The first op amp in the circuit (an OPA2134). It worked normally but had become very noisy.

Has anybody ever struck an OPA2134 with that kind of partial fault, or have I just been unlucky? Regards, Felix.

Could be ESD induced damage... I have had this experience with other fet op-amps and have deliberately zapped TL072 (another op-amp with jfet inputs) with a Keytek ESD gun and had them fail with similar symptoms.

Hi I wonder if you are still on the scene to give me some help to identify the sound fault you had to see if its the same as mine.
I have an audio file that might help, is it the same as your noise?
