So, I replaced the transformer winding... it was actually two windings, one for -12V and one for the ground side of the driver regulator circuit. After replacing this, I now get proper voltage from the driver circuit and the noise from the output is gone.
The whole process was much longer and more adventurous that just this... but that's the short story.
