You'll have to determine if there is a hole in the insulator or a burr on the heatsink or transistor.
I'd suggest reinstalling the transistor that you believe is defective to see if the problem returns.
I don't have any diagrams for this amp. All that I have use a comparator/op-amp in the power supply. The audio circuit is probably the same however.
