1. relay 12VDC, rating ampere lupa bos.
2. u can use anything small transistor for Q1,but remember of NPN. for Q2=tip32 / anything PNP.
3. sensor fan, no use. automaticaly detect from out amp.
4. NTC 10K to heatsink, heat protect. but this protect work fine without it.

What output transistors are you using,
Maybe you can get away with using less outputs on this voltage
