Sorry, I cannot figure out where the output location is for the CCS, or which parts I would minimally need to install for *just* the CCS.

Just to verify then ONLY the red parts that are crossed out need to be installed to create the CCS and R2/R3 are also not needed?

No, you'll need more parts. You need the first stage also to fix bias.

It depends of used parts but if you use my BOM ±18 V will be OK.

Can we use the CCS to bias an external op amp into class A *and* use the buffer at the same time?

It looks like we cannot, since the input will be the CCS.

Can it run from 10V if it is only going to swing less than 2V?

Tinitus - I have ordered slightly too large 'lytic caps for C6 and C7 it's an awkward fit. Apologies for this, I may get correct ones in the new year. I would advise soldering C7 inside pin before R7 maybe.
