A question, is your signal generator 600 Ohm output maybe? Could have not interfaced best with the SUT during tests so we could not know the gain exactly by compressing the sinewave somehow, but I estimate it at about 60dB for cartridge source and your SUT. Sounds like that?

It doesn't arc any more since I no longer connect the PS ground to the bottom plate of the chassis. The PCB is held off the chassis with a regular motherboard standoff.
Better late than never they say. Short answer is: HV creepage. I.e the clearances around R1 pads and other highly tensioned parts are minute to the underside ground plane for the 300V+ high voltage in the green rectification board. It arcs over the moment its associated with the high energy mains source ground. R1 acted like a fuse.

Care in spite of drinking very much wine, is raised very soon to the head but with a good woman to the side it is supported well this when sounding jazz. Yes the LAG 120A have an output impedance of 600 ohms. Yes, it haves more gain (and other important things) than the Simplistic NJFET RIAA

For ceramic fuse 6x32 380V 160mA can I use fuse holder 250VAC, right?
Ask the seller for their proper holder. 6x32mm holders have the distance and body material not to arc at 300-400V when rated for 380V fuses.
