6AS7GA as push pull output?
How does that design get away with its push-pull driver stage using an E88CC (6922, or industrial 6DJ8) with +190V plate-cathode? I know the current is low, so the E88CC is only running at about 600mW dissipation, but still, that's a lot higher than the rated max plate voltage.

Maybe something like a 6CG7 would be better there?

6AS7GA as push pull output?
6CG7/6SN7 would do well too, and then you've got the front end from the 6L6GC AB2 basically. Gain will of course be less, however.

Unless I'm mistaken 6CG7 is just a noval version of 6SN7.

It is. Just derate the heat since the bottle is smaller, otherwise they are electrically the same, although some 6CG7 (and 6FQ7) should have a shield between the two nodes, tied to pin 9.

Super handy tubes, I grab them every time I find them cheap.

My experience as well. The 6AS7G is just a very noisy tube. The 5998A is better but, impossible find in NOS condition, instead they are thrashed usually.

Can IXCP10M45S be used as a constant current sink for the LTP and second stage?

if you are going to use silicon, a 400v NPN power transistor hung off 6922 will produce 30 watts p-p from a 150v B+ and 3k transformer.
