Identifying Power Trannie from Sub?

I have a Proficient S-12 powered sub. The trannie has gone bad and i'm trying to identify the manufacturer. Nomenclature on side of this toroidal is three separate lines.

150-R0955504
LS-A10714-PT-T
LEI-4 0547

Around 250 Watt plate amp on back of sub.

Called Proficient but they are no help.

It has different taps on primary for 120-240vac

Are you trying to find a replacement?
Look at the PS caps' DC voltage rating and choose another transformer that will give close --but not above-- those rail voltages after rectification (Vrect = Vsec * 1.4). Also choose an appropriate VA (load) rating for your sub.
