Looks like a basic ABC quadrature encoder, where ABC stands for:
A = channel A
B = channel B
C = common

If no one pops up with the exact info, then just pick any ABC encoder with same amount of detent per revolution and otherwise seem to fit your electrical and mechanical needs.
