The IC in #26 could be defective but the silicone die is generally centered in the package and the IC next to it also appears to have a circular mark.
You post that the voltages on the IC are DC. Does that mean that it's pure DC or are you using a multimeter instead of a scope? Are there any waveforms on the outputs?
