Does removing that op-amp affect the voltage on the output?

Was the 4558 new?

Yes if I remove that opamp the voltage on the output drops to 0

And that opamp was new I can replace it again with a TL072 or a LM833

Are the positive and negative power supplies for that op-amp approximately equal?

I think I found the problem one of the other opamp is defective the IL4558 is defective pulling the voltage down on the other ic .
