Why do you suspect that it's defective?

Have you looked at the PS and audio feeding it to see if it was configured like an op-amp?

It was not defective but it looked like its leg was green and when I remove it to clean and resolder that leg came out of IC. So I just want to replace it. What package size is that. It look like opamp
