Could be many reasons...
To much current being drawn, to high a mains voltage being applied that pushes the core into saturation, poor AC waveform (non sine). More unlikely causes could be shorted turns internally within the windings.

Possibly. Using over sized cap values can push a transformer that is only just adequate into saturation.
If this is a problem that has only occurred following a recap then you need to carefully check that you have all the values correct and that all are fitted correctly.
