Anyone can identify this BurrBrown opamp ?

The supply voltages I measured are:
pin 7: +40V
pin 4: -40V

Quite high indeed !
Search the TI site,
but OPA2604 has highest voltage ratings of +/-24Vdc, nowhere near.

I assume it's a single opamp...
Anybody ???

I did some further searching and found high voltage opamps.
I just downloaded the datasheet for OPA445, prior to reading your reply.

It's the driver stage in an integrated amplifier.
The preamp opamps are "mutilated" as well, but since they are also BB and have supplies of +/-22V on pins 8 and 4, good chance they are OPA2604's.
