Your PCB is lacking the op-amp and pre-drive circuitry. Where does it go, or is this sort of a "universal" power amp output stage? Still, very cool and nice effort.

You have silkscreen printed over your pads.
Doesn't your board house remove silkscreen over pads on CAM import?
