In that preamp (which I used for most of the past 20 years), I used a -15V negative rail.

To answer the second question, I note that when I retired my preamp, the linestage that replaced it was the Heretical, which is indeed a cathode follower with a current source. I use a servo almost identical to that in the TF-10. And in later preamps, Berning reverted to a cathode follower with a large resistor down to a B- rail, still servoed.
