Default OPT secondary ground, stacked supplies

Hi everyone!
Looking at several dc-coupled circuits with stacked supplies I found the output transformer secondaries to be either floating or referenced to input ground.
I'd say it makes sense to connect the secondary to input ground because connected to output GND (input B+) you would have massive DC on the output.

I am wondering if the connection of the transformer to two different supplies causes any problems. Maybe my question is a bit mushy but I am struggling with the concept...

Connect it to signal (input) ground. If you connect it to a supply you'd get zapped whenever you touch the speaker terminals, this would be dangerous.
