Identify PC board in little amp...

I found this little home made amp chassis, and it has a PC board on it, which looks like it could have been from a commercial unit,,,the board has a 6AN8, and a 6AQ5 tube on it... The rectifier on teh chassis is a 6BW4...
I drew a rough diagram of the chassis/PS, and if the board is from another amp, maybe I can find a schematic for it, and get it working...
Does anyone recognize the PCB??
